As required by Indiana state law, you must first obtain a prescription from your physician before being seen by the providers at ProCare. This prescription must include the patient’s name, diagnosis, physician orders and the physician’s signature and date.
You or your physician may schedule the first appointment. On your first visit, you will be evaluated by a therapist and will receive treatment. This will last approximately 30-90 minutes. You and your therapist will develop a treatment plan based on the information gathered during the evaluation. The number of follow-up treatments will be based on your prescription, treatment plan and/or insurance authorization.
During the follow-up sessions your therapy team will treat you for approximately 30-60 minutes. Our goal is to keep you with the same therapy team. However, there may be times such as vacations, illness or scheduling conflicts, which will require you to be seen by another therapist.
Therapy Team: Our teams consist of therapists and therapist assistants. Your therapist and assistant communicate on a daily basis about your treatment plan, progress and goal status. Any questions you have in regards to your treatment or condition can be directed to either team member.
We are proud to have been selected as a clinical teaching site for many universities; therefore, you will also see students working under the direct supervision of your therapist. Please let us know if you have any questions or concerns.
Dress: Please dress comfortably in casual clothing that will not restrict your movement and can expose the area being treated. We suggest T-shirts, shorts or sweatpants, and tennis shoes.
If you are scheduled for aquatic (pool) therapy you will need a bathing suit and towel. For your safety we also recommend aqua shoes or sandals to wear to and from the locker rooms though these are not required. Lockers are also available for your use but we do not supply locks.
Attendance for all scheduled appointments is necessary for your treatment to be effective. All attempts will be made to accommodate your schedule. If you are unable to attend a scheduled appointment, we ask that you give as much notice as possible. A message can be left on voice mail after hours. Our clinic hours are:
